Position Overview:
We are currently seeking a passionate and dedicated Program Manager with hands-on operational coordination, the Manager will complement the team by ensuring the program delivery aligns with aged care standards (CHSP, SAH, DVA, IHP), supports community well-being, and reflects Auscare values. This position offers a rewarding opportunity to make a meaningful difference in the lives of those we serve.
Key Responsibilities:
Supervise and support the coordinators, ensuring consistent delivery and continuous improvement of participant assessment processes and personalised care planning, alongside broader operational responsibilities
Supervise and support Coordinators, Engagement & Events Lead
Drive and oversee organisational strategy and long-term planning in consultation with the management team
Ensure governance, risk management, and compliance across all Auscare programs, aligning with relevant legislation and funding bodies
Maintain strong knowledge of CHSP, aged care reforms, and Quality Standards
Plan, coordinate, and facilitate regular team meetings to support the person-centred goals and outcomes, reflective practice, and risk management
Coordinate DEX reporting and contribute to CHSP audits, reviews, and reports
Coordinate all internal and external audits , reviews and corrective actions
Liaise with funding bodies, community partners, stakeholders, and collaborators
Represent Auscare at external forums, local networks, and government consultations
Lead financial planning and accountability, including budget development, forecasting, and resource oversight in collaboration with the accounting team
Support marketing, communication, and social media strategy in collaboration with Auscare Marketing team
Qualifications:
Tertiary qualifications in Management, Allied Health, Registered Nurse or a related discipline; or extensive experience in management in Support at Home or Community Care or Aged Care
Experience in multidisciplinary approach to assessment and care planning in home care setting
Strong communication and interpersonal skills.
Empathy, patience, and a genuine passion for improving the lives of others.
